Health information

Your protected health information (PHI) is handled under HIPAA and is collected only inside our secure patient portal (Healthie), never through the forms on this website. Please do not include symptoms, diagnoses, medications or other health details in website forms. Our separate Notice of Privacy Practices, available on request and in the patient portal, explains how we use and protect PHI.

Information we collect

1. Personal information: when you contact us, request an appointment, download a guide or apply for a job, we collect what you enter, such as your name, email address, phone number, insurance carrier and preferred visit type.
2. Non-personal information: we use Google Analytics and Google Ads to understand how visitors use the site in aggregate (pages viewed, device type, general location). We send these tools event names such as “booking button clicked” only, never what you type into forms. We have turned off Google Signals and ad personalization signals, and we do not use Facebook/Meta tracking pixels.

Sharing and disclosure

We do not share, sell, rent or trade your PII to third parties for marketing purposes.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the categories in this privacy policy exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

• Service providers that perform functions for us, such as our website host (which delivers form submissions to us), scheduling and patient portal software, and email providers. They are contractually obligated to protect your information.
• Legal requirements: when required by law or to protect our rights, property or safety.

Data security

We use reasonable administrative, technical and physical safeguards to protect your information. No method of transmission or storage is 100% secure, so we cannot guarantee absolute security.
