For doctors & providers

Refer a patient for nutrition care

You don't always have time for in-depth diet and lifestyle counseling. Our registered dietitians do, and we keep you updated on your patient's progress.

Three ways to refer

  1. Fax the referral form to 866-850-2765. Our patient coordinator contacts your patient right away to schedule.
  2. Download our physician referral form (PDF) and our one-page provider info sheet (PDF) for your team.
  3. Call us at 678-250-3438.

Medicare covers medical nutrition therapy for diabetes and chronic kidney disease with a physician referral. Please include the diagnosis code.

Why refer to us

  • Better outcomes: targeted nutrition care supports blood sugar, cholesterol and weight goals.
  • Prevention: lifestyle counseling that may reduce the need for costly interventions.
  • Value-based care: measurable progress that supports quality metrics.
  • Coordinated care: visit notes faxed back to your office after every visit.
  • Access: in-person in Suwanee and Lawrenceville, video in 25 states, in eight languages (Spanish with our on-staff interpreter).

Let's work together

Two ways to partner with us, beyond referrals:

Book a lunch or coffee chat

Host a dietitian at your office (Georgia)

Practice details only. Please don't include patient information in website forms; send patient referrals by fax.

Lunch, coffee or a virtual chat

Let's talk about how we can help your patients. We'll bring lunch or coffee to your office in Georgia, or meet by video if your practice is in a state we cover.

  • Meet the dietitian who would see your patients
  • How referrals, insurance and visit notes work
  • Conditions we help with and languages we speak
  • About 20–30 minutes, at a time that suits your team

Request a meeting

A dietitian in your office (Georgia)

For practices in Georgia, we can send a registered dietitian to see your patients at your office. Your patients get nutrition care in a place they already know, and you keep care under one roof.

  • A set clinic day at your office, with at least 5 patients scheduled
  • We see the patients at your facility and send visit notes to your team
  • We handle the billing with the patient's insurance

Send us your details and we'll share more information and next steps.

Ask about an in-office dietitian

What to include on the referral

  • Patient name, date of birth and phone number
  • Insurance information (front and back of the card, if available)
  • Diagnosis with ICD-10 code(s); required for Medicare
  • Recent labs, such as A1C, lipid panel, eGFR or TSH, when relevant
  • Reason for referral and any goals or diet orders
  • Referring provider name, NPI and signature

What happens next

  1. We receive your fax at 866-850-2765.
  2. Our patient coordinator calls your patient right away, with up to three attempts by phone and text.
  3. We verify insurance benefits before the first visit.
  4. The dietitian completes a full nutrition assessment and builds a plan with your patient.
  5. We fax visit notes back to your office after every visit and partner with you on care.

Questions from referring offices

How quickly will you contact my patient?

Our patient coordinator reaches out as soon as the fax arrives during business hours. We make three attempts by phone and text to schedule.

Which insurance plans do you accept?

We are in-network with 15 plans, including Medicare, Aetna, Anthem Blue Cross Blue Shield, Cigna, UnitedHealthcare and Humana. We verify benefits before the first visit.

What does Medicare cover?

Medicare Part B covers medical nutrition therapy for diabetes and chronic kidney disease (and for 36 months after a kidney transplant) with a referral from a physician or qualified practitioner: 3 hours in the first year and 2 hours each year after. Additional hours can be covered with a new referral when the patient's condition, treatment or diagnosis changes.

Will you send updates?

Yes. We fax visit notes back to your office after every visit and work in partnership with you on your patient's care.

Do you see patients who don't speak English?

Yes. Our dietitians speak several languages, and a Spanish interpreter on our staff can join visits.
